Position Overview Case Manager II – Casa Canal Permanent Supportive Housing Program at SHELTER, Inc. Location: San Rafael, CA. Full Time, Non‑Exempt. Compensation: $30.75‑$33 /hr.

Responsibilities

Screen prospective program participants for eligibility and provide complete information regarding services, expectations, rights, and responsibilities.

Obtain and review necessary case acceptance authorizations, consents, and income verification documentation to determine eligibility for program and services.

Complete assessments including HMIS Intake, VI-SPDAT, and Housing Retention Assessment to evaluate participant strengths, needs, and risk indicators.

Conduct interviews, obtain resources, and access information to formulate a comprehensive case plan.

Demonstrate knowledge and understanding of participant health and social presenting risks and apply appropriate risk stratification and planning.

Document accurately in the established record system and communicate participant care plan with support staff.

Perform routine comprehensive, independent reassessment of participant status and progress toward achievement of plan goals.

Apply effective follow‑up dates to care plan interventions and maintain accurate, up‑to‑date documentation.

Provide post‑placement and/or post‑exit support as indicated by program or participant needs and maintain post‑placement contact at 1, 2, 3, 6, 9, and 12 months.

Participate in internal and external case conference/coordination meetings and keep current on best practices.

Maintain accurate, up‑to‑date documentation in the established case management record system and comply with data integrity and security policies.

Other duties as assigned.

Qualifications

Bachelor’s degree in social services or related field, or equivalent experience.

Experience working with low‑income and/or homeless populations.

Proficient computer skills, including Microsoft Office Suite.

Proficient typing skills and ability to work flexible hours.

Access to reliable personal transportation and a valid driver’s license.

Must successfully pass a criminal background check and TB test clearance.

Preferred Qualifications

Master’s degree with emphasis in social services, psychology, or related field.

Experience in supportive housing programs.

Bi‑Lingual English/Spanish.

Key Proficiencies You’ll Need

Knowledge of social service resource systems and self‑help intervention strategies.

Independently counsel populations with mental health, substance abuse, and homelessness.

Knowledge and skills in handling substance abuse and mental health issues.

Knowledge of public benefits and financial resources available in the community.

Ability to develop relationships utilizing motivational interviewing techniques.

Crisis intervention and conflict resolution skills, including harm reduction and trauma‑informed care.

Knowledge of family budgeting and money management.

Thoroughness and accuracy with data collection, entry, and quality control in a web‑based database.

Strong oral/written communication, listening skills, and professionalism.

Self‑motivated and accountable for time and agency resources.

Well organized and able to manage multiple assignments to meet deadlines.

Position Requirements

Access to reliable personal transportation and valid driver’s license.
