Full-Time, 40 hours per week, Monday through Friday, 8:00 a.m. to 5:00 p.m., with flexibility to work later as needed. The

RN Case Manager - Care Ally

collaborates with physicians, patients, and families to promote quality outcomes for patients with complex medical needs. Responsibilities include: Analyzing data from predictive modeling tools to identify eligible patients. Managing a caseload of patients with complex needs, completing assessments and documentation per health plan requirements. Developing individualized care plans with prioritized goals and interventions. Coordinating healthcare services with providers. Maintaining comprehensive logs and summaries of active cases. Acting as a resource for staff, physicians, and patients. Participating in policy evaluation and continuing education. Performing additional duties as assigned. Qualifications

Licensure/Certification: Active Registered Nurse license in Illinois in good standing. Case Management certification preferred. Current CPR (BLS) certification. Education: Associate’s degree in nursing required; Bachelor’s preferred. Experience: 2-4 years in primary care or hospital setting as a RN. Previous case management experience preferred. The salary range is $79,040-$104,000, with actual pay based on experience, skills, education, and location. Additional incentives may apply.
