Current licensure to practice as a Registered Nurse in North Carolina required At least 5 years of bedside nursing experience (will consider 4 years if ED/ICU nursing experience) Certification in the appropriate specialized field within 2 years of being in the position Excellent communication skills Adaptability Ability to manage technology effectively Microsoft tools experience Strong critical thinking and communication skills Ability to work independently Job responsibilities include, but are not limited to: Meeting with patients in the hospital to explain Hospital at Home Program and obtain consents Coordinate medications and equipment for at-home care and assist in coordinating transportation; ensure smooth transition from hospital to home Coordinate with Case Management, Attending Physician Coordinate patient care to increase efficiency and quality of care Serve as patient advocate and provide supportive, personalized care Act as liaison to ensure patients and families receive resources and services Provide language-specific education, psychosocial support, and resources for newly diagnosed patients and families; coordinate referrals as needed Educate patients and families about diagnosis, work-up, treatments, and follow-up; discuss support services and survivorship Serve as liaison between departments and coordinate new patient referrals and scheduling Identify barriers to care (insurance, transportation) and refer to needed resources Identify trend barriers to timely care and conduct service recovery as needed Responsible for patient satisfaction Physical Requirements Ability to lift up to 25 pounds; occasional stooping, kneeling, crouching; may require sitting/standing for long periods Patient Population Served Demonstrates knowledge of growth and development principles and provides care appropriate to patient age, culture, developmental stage, and special needs Protected Health Information Limit PHI access to information reasonably necessary to do the job; share information on a need-to-know basis PHI access controlled via user ID and password Machines, Tools, and Equipment Proficient with copier, telephone, computer, software (Word, Excel, PowerPoint), and internet usage Our Commitment to You Advocate Health offers a comprehensive Total Rewards package including benefits, competitive compensation, retirement offerings, and career development opportunities.
