The Role The
Peer Recovery Specialist
plays a key role in working closely and collaboratively with other Specialists to coordinate patient care, keep the patient engaged with services, address issues related to relapse, and communicate together on patient needs. This role works under general direction of the Hub Administrator and Hub Care Manager in providing services to the Multicare Health System Hub & Spoke program, and as a part of the Ideal Option team. Additional responsibilities include:
Works closely and collaboratively with Hub & Spoke Care Navigators to provide coordinated care
Facilitates ongoing coordination of follow-up care to treatment and recovery support services (e.g., transportation, housing, employment, mental health and/or chemical dependency assessments, primary care, food and clothing resources) offered at the Hub and Spoke sites and various partnered agencies
Performs individual patient screening to determine needs and lacking resources
Provides medication assisted treatment (MAT) education for patients
Performs patient charting in the EMR system
Provides support for individual’s self-management and engagement
Performs community outreach efforts to various agencies, as well as underserved communities
Has a knowledge of substance use disorders and their effects on individuals, families, and communities. Is able to communicate effectively with patients and within the organization and community agency members. Is able to adapt to individual needs. Good at problem solving, decision making and upholds the principles of ethical conduct.
