Monday-Friday, 40 hours per week Ideal Option has been working on the front lines of the opioid epidemic since our first clinic opened in 2012. Performs outreach and education to eligible individuals Performs individual patient screening to determine needs and resources and eligibility for services as needed Tracks and enters demographic and related information for care coordination Complete weekly and monthly reporting as needed Facilitates ongoing coordination of follow-up care to treatment and recovery support services (e.g., transportation, housing, employment, in-patient facilities, detox, mental health and/or chemical dependency assessments, primary care, food and clothing resources) Develops community partner relationships Attends trainings and meetings as required Our ideal candidate
Has a knowledge of substance use disorders and their effects on individuals, families, and communities. Is able to communicate effectively with patients and within the organization and community agency members. Is able to adapt to individual needs. Good at problem solving, decision making and upholds the principles of ethical conduct.
