What You’ll Do The Nurse Manager is a key leader within the multidisciplinary team, overseeing the healthcare needs of individuals in a community setting.

Clinical Care & Treatment

Assess health needs, develop care plans, and administer medications/treatments per regulations (DPH, CARF).

Provide direct support, including ADLs, mobility assistance, and community integration.

Care Coordination & Advocacy

Liaise with physicians, specialists, and stakeholders to ensure seamless healthcare delivery.

Coordinate hospital admissions, discharges, and follow-up care.

Compliance & Safety

Maintain medication logs, infection control, and safety protocols.

Ensure program adherence to funder, state, and accreditation standards.

Stakeholder Communication

Deliver timely health updates to families, guardians, and funders.

Document and report care outcomes accurately.

Staff Leadership & Training

Train and mentor staff on medical protocols and person-centered practices.

Foster a supportive environment using Positive Behavior Supports (PBS).

Operational Flexibility

Participate in on-call rotation (24/7) and provide holiday/weekend coverage as needed.

Assist with day/residential program coverage during staffing gaps.

What You Bring

Required: Active LPN license in MA, driver’s license, and reliable car.

Preferred: Active RN license in MA, driver's license, and reliable car.

Experience: Supporting mental health needs (personal or professional).

Skills: Clear communication, basic computer use, and teamwork.

Language: Must be fluent in English, Multilingual a bonus!
