Must have four years of Pediatric experience and LPN licensure. Position Purpose:

Assesses, plans, implements, and coordinates care management activities based on member needs to provide quality, cost-effective healthcare outcomes. Develops or contributes to the development of a personalized care plan/service plan for members and educates members and their families/caregivers on services and benefit options available to improve health care access and receive appropriate high-quality care through advocacy and care coordination. Develops or contributes to ongoing care plans/service plans, identifying providers, specialists, and community resources needed for care. Coordinates and manages between the member, family/caregivers, and care provider team to ensure services are accessible in a timely manner. Monitors progress towards care plan goals and collaborates with healthcare providers for revisions or addressing member needs. Provides psychosocial and resource support to members/caregivers, and care managers to access local resources such as employment, housing, food, etc., based on assessments. Identifies problems/barriers for care coordination and implements appropriate interventions for members with higher care needs. Reviews referral information and assessments to develop or assist in developing appropriate care plans for members with higher needs. Collaborates with healthcare partners to facilitate member care and determine if revisions are needed. Maintains all member information and activities to ensure compliance with regulations. May perform on-site assessments and collaborate with providers or resources. Provides education on procedures, healthcare instructions, care options, and benefits. Serves as a Subject Matter Expert (SME) and preceptor for new staff. Performs other duties as assigned to meet member and business needs. Education/Experience:

Bachelor’s degree and 4–6 years of related experience. Clinical licensure as a Nurse preferred. Equivalent experience may be considered. License/Certification: Current state’s clinical license preferred. For CMS Florida licensed practical nurses: minimum four years pediatric experience, or a Master’s in social work with at least one year of pediatric care experience. Up to 80% local travel may be required. Pay Range: $26.50 - $47.59 per hour Centene offers comprehensive benefits including health insurance, 401K, stock plans, tuition reimbursement, paid time off, and flexible work arrangements.
