Responsibilities NAVIGATOR SPECIALITY – MATERNITY
The Maternity Specialty Navigator works primarily with high-risk expectant mothers, delivering comprehensive support for access to quality care throughout their pregnancy, delivery, and postnatal journey, particularly addressing any existing chronic health conditions to reduce risk and promote overall well-being.
Managing appointment scheduling to ensure follow through
Transportation assistance to improve access to care by arranging transportation for or driving patients to and from their appointments in navigator’s car
Prescription coordination to enhance medication and treatment compliance and medical outcomes
Provide individualized Peer Support services
Adherence to Gellert Health’s policies, procedures and protocols
Complete documentation of patient interactions in our EHR system
Collaboratively working with the Team on cross-coverage needs
Fulfill additional responsibilities as assigned by Direct Supervisor

Ability to navigate electronic medical record & perform other basic computer skills

Problem Solving skills and Resourcefulness
Solid understanding of physical, medical and behavioral health services
Able to work independently and collaboratively within a team
Requirements
Associate degree or bachelor’s degree, or relevant work experience
At least 21 years old
39 months clear Motor Vehicle Record
Current Arizona driver’s license
Auto insurance
Clean and safe four-door car with ability to drive patients to and from appointments
Experience working with culturally diverse, chronically homeless, and vulnerable individuals, in medical and behavioral health services
Experience:
Maternity Specific patients/cases: 1 year
Healthcare: 1 year
Behavioral health: 1 year
Social Services Occupations: 1 year
Bilingual – Spanish (preferred)
Training and Evaluations
2 to 3 weeks New Hire Orientation and Training, including shadowing in the field
Weekly individual supervision and ongoing training and support with oversight of job performance
90 Day Review & Annual Evaluation
