POSITION SUMMARY:

Under the direction of the Transitional / Emergency shelter Manager, the Case Manager participates in all programs and activities for staff and clients. The Case Manager empowers survivors by following the agency’s mission and goals. The Case Manager supports the Program Manager to ensure the success of the Shelter Program. Overview

The Case Manager supports program administration, staff training, and the coordination of case management and social service functions to assist survivors in achieving housing stability and access to necessary resources. Responsibilities

Assisting with program administration, the training of staff and representation of the program internally and externally Ensuring that evidence-based practices are utilized through example and by providing training Conducting individualized needs assessments and collaborating with clients to develop permanent housing plans that address barriers to obtaining and maintaining stable housing Providing ongoing case management support to assess progress and ensure housing plan goals are met or adjusted as necessary Coordinate the integration of social services/case management functions into the participants planning process Regularly review all participants’ files from all the program Keep detailed records of every case, every referral and every contact with other agencies on behalf of the clients Help clients work through urgent needs; act as an advocate and facilitator; work in partnership with survivors Relay information to Shelter Manager on a weekly basis and keep detailed records of these meetings Network with other agencies to stay informed about services and resources; refer clients to other programs when appropriate Follow up with other agencies that have commitments to assist clients with their needs Respect confidentiality at all times Remain in contact with clients until needs are met and the case is closed Represent the agency at community meetings and special events as assigned Assist in conducting orientations to new volunteers/interns, as requested Additional duties and special projects may be assigned as necessary Minimum Qualifications

Bachelor’s degree in human services or related field Certification in the 40-hour domestic violence victim services training required by the State of California or willing to obtain within first 3 months of employment Bilingual required in English/Spanish Experience working in shelter setting Good written and oral communications skills Possess basic computer knowledge and skills Vehicle required with valid CA driver’s license and auto liability insurance Has not been a client of Su Casa or any other DV Shelter nor been in a domestic violent relationship for one year Clear state and Department of Justice criminal background check Sensitivity and supportive to individuals and families in crisis Able to work as a team member with exemplary organization skills, good interpersonal skills, strong leadership qualities, and good judgment Must be fully vaccinated against or willing to undergo weekly testing for COVID-19 after providing completed accommodation paperwork
