Job Summary The Nurse Clinician, Hospital at Home (NC- HaH), has knowledge and skills necessary to meet standards for professional practice specified in the Texas Nursing Practice Act and by Harris Health Nursing Service. The practice patterns of competent generalists are characterized by the application of standards, principles, and algorithms to clinical situations. The Nurse Clinician, Hospital at Home, is responsible for providing high-quality, at-home acute care. The NC-HAH independently and safely manages patients with problems common to patients in his/her assigned clinical area; recognizes when he/she needs assistance and seeks appropriate resources. The NC-HAH coordinates, facilitates, and participates in the development and implementation of facility enrollment of the eligible HaH patient system-wide utilizing the appropriate eligibility criteria. The NC-HAH develops, organizes, and facilitates the delivery of patient-centered care in the patients' home by prioritizing patients' needs for in-home acute care delivery; utilizing a scheduling system for the efficient, timely, spatially and geographically-organized acute care home visits and point of care delivery; and accessing and using evidence-based resources and professional standards related to carrying out roles and responsibilities. The NC-HAH participates in the quality improvement projects which involve collection of data for trends related to all clinical screening, referral criteria, and other clinical/business indicators as directed by leadership. The NC-HAH identifies problems and applies generated information to health care practice with the goal of improving clinical outcomes. The NC-HAH role models professionalism and promotes a customer-friendly environment by utilizing Service First behaviors while interacting with patients, families, and staff members.

Degrees

Bachelor's degree in Nursing

Licenses & Certification

Registered Nurse: In the state of Texas

Related Specialty Certification: Other certificates as required by unit/service

Basic Life Support: Current Basic Life Support from an American Heart Association (AHA) or Red Cross approved program

Current and unencumbered Texas Driver's License

Work Experience

2 Years of Experience: Clinical experience in an inpatient setting, home health and or community visit setting

1 Year of Experience: Home health/hospice and/or med-surge

Language
