Description U.S.VETS is the largest nonprofit organization fighting America’s veteran homelessness crisis. The Case Manager II supports veterans in achieving independence by coordinating comprehensive services and managing individual action plans.

Responsibilities
Assess veterans referred to the program for barriers in housing, employment, education, finance, sobriety, legal, family/social, and medical/psychiatric areas.
Coordinate care with VA personnel and community partners for eligibility and health services.
Facilitate client intake, compile files, and ensure eligibility documentation is complete.
Collaborate with clients to complete Individual Action Plans (IAP) and review monthly updates.
Maintain confidential client files, documents, and data entry into the HMIS system.
Document progress notes in DAP format and conduct home visits to support client health and welfare.
Assist in data collection, reporting, and quality‑management activities.
Provide life‑skills classes and specialized services to veteran subpopulations (e.g., female, disabled, Iraq/Afghanistan).
Conduct random drug and alcohol screenings and uphold HIPAA confidentiality.
Maintain time sheets, mileage, and expense logs; drive company vehicle for client transportation.
Perform additional duties as required by the program.
Program‑Specific Duties (examples)
Transitional Housing Clients:
Manage daily contact, bed checks, rent compliance, and locate appropriate housing units.
Permanent Housing Clients:
Secure and maintain permanent housing, coordinate educational enrollment for children.
Workforce Clients:
Assist with employment development, resume writing, job placement groups, and job‑related training.
SSVF Clients:
Provide rapid rehousing, housing stability plans, home visits, and maintain VA compliance.
Aftercare Programs:
Monitor housing stability, coordinate education and referral services, and attend case conferences.
Incarcerated Veterans Transition Program:
Assist with program assessment, develop employment plans, and coordinate jail-based services.
Nursing Credentials:
Oversee infection control, conduct health screenings, maintain credentials, and collaborate with healthcare partners.
Legal Services:
Educate veterans, assess legal needs, coordinate services, and manage grant compliance.
Qualifications
Bachelor’s Degree in social work, psychology, counseling, or a related field (preferred).
Strong oral and written communication skills.
Professional appearance and demeanor.
Ability to work independently and within a team.
Proficiency in Microsoft Office 365, email, and internet research.
Preferences
At least two years of experience in social services or a related field.
Experience with homeless and/or veteran populations.
Certification in Substance Abuse Counseling.
Experience facilitating support groups.
Valid driver’s license with company insurance requirements and training completion.
