Responsible for providing high quality in-home case management services to Veterans and families housed through the Rapid Rehousing & Shelter Diversion Program. The case manager assists clients in developing goals that lead to housing stability, monitors progress toward these goals, and provides structure, direction, and service linkage for clients in order to assist them in sustaining permanent housing and achieving their life goals. Qualified candidates must possess a clear understanding of the service delivery system and the resources available for veterans and their families.
Primary Duties
Develops with each client a detailed Housing Stability Plan centered on goals that assist in maintaining stable housing. The plan will include specific goals established but not completed while residing at the shelter, as well as new goals centered around housing stability, with action plans and timelines for areas such as:
Financial needs and Budgeting
Employment and Educational Services
Family Support Services
Landlord/Tenant Relationship and Community Living
Provide services including community outreach, acting as a liaison with tenants and landlords, and mediating complaints/concerns to proactively prevent returns to homelessness.
Connect Veterans to VA health care benefits or community health services when VA care is not available.
Serve as a liaison between the SSVF grantee and VA or community medical clinics, assisting Veterans with complex needs to access health care services and adhere to health care plans.
Provide individual life skills counseling and support.
Conduct a minimum of weekly in-home visits to provide structure and guidance.
Monitor and facilitate linkages for clients in health, mental health, education, employment, housing, benefits, and parenting support.
Process Temporary Financial Assistance requests.
Collect monthly statistics and report to the Director as required.
Secondary Duties
Stay updated on community services and maintain a referral database.
Participate in client registration and processing via Rosie (HMIS).
Ensure clients understand, sign, and comply with the Participant Service Agreement.
Provide crisis intervention as needed.
Maintain basic knowledge of community resources for veterans, especially VA services.
Represent the program’s mission to various groups and organizations.
Attend and support special events and meetings.
Uphold the organization’s vision, mission, and values.
Support a welcoming environment for volunteers and collaborate effectively with them.
Perform other duties as assigned within the scope of the position.
Qualifications
Bachelor’s Degree in Human Services or related field.
At least 2 years of experience working with homeless or materially poor populations, preferably in a housing program.
Flexible schedule availability, including weekends and evenings.
Knowledge of confidentiality and respectful communication across diverse backgrounds.
Ability to work effectively as part of a team.
Physical and Environmental Conditions Primarily indoor work with medium physical effort, including lifting up to 50 pounds occasionally.
