Canyon Ridge Hospital is currently seeking a compassionate and talented
Case Manager
to join our team. At Canyon Ridge Hospital we understand that not all wounds are visible. Depression, anxiety, trauma, PTSD, addiction, and suicide affects millions of individuals and families. Canyon Ridge is a 157-bed acute, locked psychiatric facility offering services to adolescents ages 13-17, adults and older adults. We believe in an individualized, patient-focused approach to diagnosis and treatment. For some, anxiety or drug dependence looks different from others with the same diagnoses, and in some cases, behavioral health and substance use disorders occur simultaneously. In these cases of dual diagnosis, we treat the patients in the best way possible given their specific needs. Responsible for providing quality case management, utilization review activities and psychotherapeutic services to psychiatric patients and their families, spouse, significant other; to serve as a member of interdisciplinary team supporting the organization's treatment program and philosophy and assure the deliverance of quality treatment to psychiatric patients and their families. Growth and development opportunities within UHS and its subsidiaries Qualifications Masters degree in Social Work 2 years of experience in inpatient setting preferred; Written and oral communication skills, knowledge of utilization management & skills in case management, time management, discharge planning and crisis intervention common to acute psychotic as well as to non-violent crises intervention practice. Knowledge of acute adult psychological disorders Assessing clients' needs and developing individualized care plans. Coordinating with healthcare providers, social services, and other stakeholders. Monitoring and evaluating clients' progress and adjusting care plans as needed.
