Overview
Statewide Nurse Practitioner or PA – Optum HouseCalls – Tennessee. This role provides annual in‑home clinical assessments to patients across Tennessee as part of Optum’s Home and Community Care team. The program focuses on preventive care, identifying health risks, coordinating follow-up care, and improving overall health and well‑being. The role supports a flexible schedule over a 7‑day work week with no on‑call, weekends, or holidays required. The position requires travel across the entire state (approximately 85% of the time). All travel expenses are covered. There is a base salary plus incentives. Primary Responsibilities
Conduct in‑home assessments including evaluation of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical examinations. Perform evidence‑based practice screenings including point‑of‑care testing as appropriate. Identify diagnoses to be used in care management and active medical management to inform treatment. Formulate a list of current and past medical conditions using clinical knowledge and assessment findings. Communicate assessment findings to inform the member's primary care provider of potential gaps in care. Identify urgent and emergent situations and intervene appropriately. Educate members on topics such as disease processes, medications, and compliance. Required Qualifications
Active NP license or ability to obtain by start date; licensure must be unencumbered with no open disciplinary actions, probation, or restrictions on practice. For NPs: must have approval to practice autonomously or without formal supervision where applicable; if eligible but not yet approved at hire, must apply within 1 month and pursue autonomous practice within 3 months of eligibility. For PAs: ability to work under a Collaborative Agreement in applicable states. Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult‑Gerontology Primary Care, or Adult‑Gerontology Acute Care, or the ability to obtain national certification and/or NP license in the state of assignment by start date. Ability to complete physical requirements, including lifting up to 30 pounds, navigating stairs and various dwelling conditions, and performing physical assessments while seated, standing, or kneeling. Reliable transportation to travel up to 85% within licensed states (with notice per business needs) to complete home visit assessments. Preferred Qualifications
1+ year of clinical experience at their highest level of education in a clinical setting. Experience in Gerontology, Cardiology, Internal Medicine, or Endocrinology. Home health care or home visit experience. May require additional licensure in other geographic areas. Salary, Benefits, and Eligibility
Pay is based on factors including local labor markets, education, work experience, and certifications.
