Sunrise of San Rafael . The Medication Care Manager is responsible for providing the highest degree of quality care and services by administering medication and treatments in a safe organized manner. Responsibilities include administration of medications, documentation of medication administration, and providing resident care while demonstrating the Sunrise mission to champion the quality of life for all seniors in accordance with laws, standards, and Sunrise policies. Provide the highest degree of quality care and services by administering medication and treatments in a safe, organized manner. Review, read, notate, and initial the electronic health record to document and learn about pertinent information about residents. Receive medication updates from the Resident Care Director (RCD) or Wellness Nurse. Administer, assist with, and observe medications and treatments for each resident using the medication administration record and the Six Rights of Medication Pass (Right resident, medication, dosage, time, route, right to refuse). Ensure medications are passed according to times and using the mobile med cart. Document and initial as medications are given and ensure appropriate documentation for refusals or missed doses. Maintain confidentiality of all resident information, including medication details. Report resident concerns and unavailable medications to the RCD or Wellness Nurse. Restock medication cart after all medication passes. Assist in checking medication regardless of packaging system. Assess residents to determine the need for "as needed" medication and document/report to supervisor. Count narcotics with another MCM or Lead Care Manager (LCM) each shift and report discrepancies. Maintain and clean the Medication Room, medication carts, and treatment carts; ensure neateness and proper medication availability and expiry management. Follow refill process for medications and notify RCD or Wellness Nurse if a new prescription is needed. Help residents maintain independence and promote dignity and physical safety, adhering to Sunrise Principles of Service. Respond empathetically to residents, considering each resident’s unique communication patterns, history, and needs. Practice standard care precautions of cleanliness, hygiene, and health. Notify RCD of any resident and/or family concerns. Attend and participate in daily Cross Over meetings facilitated by the LCM. Participate in the development of the Individualized Service Plans (ISP). Risk Management and General Safety
Ensure the community complies with regulations pertaining to occupational health and safety; promote risk management programs and policies. Practice safety procedures including PPE, fire safety, SDS, and lockout/tagout where applicable. Report accidents/incidents and unsafe/hazardous conditions immediately. Keep cords, carts, equipment, and hazards out of walkways and in compliance with fire codes. Adhere to infection control techniques and proper disposal of bio-hazard materials. Report occupational exposures to blood, body fluids, infectious materials, sharp objects, and hazardous chemicals immediately. Ensure oxygen tanks are stored safely and maintained at prescribed levels. Training and Team Contribution
Participate as a member of the team and work toward team goals. Model Sunrise Credo in daily interactions and contribute to engagement initiatives. Attend regular meetings and trainings as directed by supervisors and coordinators. Maintain compliance with required trainings and standards. Perform other duties as assigned. Core Competencies
Ability to make decisions in the resident’s best interest. Remain calm in difficult situations and manage multiple priorities. Strong written and verbal communication skills; good judgment and problem-solving abilities. Experience and Qualifications
High School diploma/GED; additional regulatory requirements may apply by state/province. Maintain applicable certifications (e.g., CPR and First Aid) as required by Sunrise and regulations. Must be at least 18 years of age. Previous experience working with seniors preferred; a desire to care for seniors. Compensation and benefits details will be provided during the recruitment process.
