Pay Range Base pay range: $22.10/hr - $27.70/hr

Job Overview The Lead Medication Care Manager is responsible for providing the highest degree of quality care and services by administering medication and treatments in a safe, organized manner, leading the assigned tasks of Medication Care Managers. The role includes administering medications, documenting medication administration, leading and resident care, and demonstrating the Mission for Sunrise Senior Living: “to champion the quality of life for all seniors.”

Lead Medication Care Responsibilities

Administers medication and treatments in a safe, organized manner, including leading the assigned tasks of Medication Care Managers.

Performs the duties listed within and leads Medication Care Managers in the duties listed in this job description.

Reviews staff schedule, ensures appropriate staffing coverage in the absence of the Coordinator/RCD; contacts alternative team members as needed to fill staffing vacancy.

Monitors attendance and daily break schedules.

Demonstrates working knowledge of the daily variable staffing model and resident service levels.

Leads daily Cross Over meetings, encourages active participation and teamwork within the changing shifts.

Participates in resident assessments.

Receives and verifies delivered medications and faxes of new orders from the pharmacy.

Medication Administration

Reviews, reads, notates in the electronic health record to document and learn about pertinent information and any resident’s physical and behavioral changes.

Checks for medication updates with Resident Care Director (RCD) or Wellness Nurse.

Prepares medications and treatments for each resident’s medication administration with attention to demonstrating and following the “Six Rights of Medications Pass” (“Right” resident, medication, dosage, time, route, right to refuse).

Administers medications and completes treatments to residents using the Medication Administration Record (MAR) and the Treatment Administration Record (TAR).

Completes medication passes within allowed timeframe utilizing a mobile medication cart.

Maintains confidentiality of all resident information including resident medication among other residents.

Documents legibly and reports all resident concerns, including unavailable medication, made while administering the medication.

Maintains organization and cleanliness of medication cart.

Restocks medication cart with cups, spoons, etc after all medication passes are completed.

Assists in checking medication refill status.

Assists with obtaining monthly resident weight and vital signs.

Assesses the residents to determine need for “as needed medication” and appropriately documents and reports to supervisor.

Ensures completion of shift narcotic count per Sunrise policy.

Prepares medications for resident leave of absence.

Completes peer review medication pass observations as directed.

Maintains and cleans the Medication Room, med charts, treatment carts for neatness, cleanliness, availability of medications and expired medications.

Risk Management and General Safety

Partners with community team to ensure community is in compliance with national/provincial regulations pertaining to occupational health and safety requirements and promotion of Risk Management programs and policies; adherence to safety rules and regulations.

Practices safety procedures at all times including Personal Protective Equipment (PPE), fire extinguishers, Safety Data Sheets (SDS) and Lockout Tagout procedures.

Reports all accidents/incidents immediately.

Reports all unsafe and hazardous conditions/equipment immediately.

Ensures any cords, carts, equipment and other hazards are kept out of the way at all times, not blocking exits and in compliance with fire codes.

Complies with all infection control techniques, placement of bio-hazard containers and removal techniques, procedures and policies.

Understands and practices the proper method of attending to and disposing of and the possibility of exposure to blood borne pathogens, bodily fluids, infectious waste, sharp sticks and hazardous materials.

Reports occupational exposures to blood, body fluids, infectious materials, sharp sticks and hazardous chemicals immediately.

Ensures oxygen tanks are stored safely, exchanges guest/residents tanks when empty and monitors to make sure liters of oxygen are at prescribed levels.

Maintains universal care precautions of cleanliness, hygiene and health standards.

Resident Care Communications

Communicates effectively to supervisor and documents any information regarding the resident’s physical, emotional and cognitive changes.

Assists Resident Care Director (RCD) and Wellness Nurse with delegated nursing tasks.

Notifies supervisor of any resident and/or family concerns.

Signs off on completed MAR/TAR.

Maintains communication with designated family contact.

Provides input into the resident’s Individualized Service Plan (ISP).

Attends regular meetings; Town Hall, Department Team Meetings, Cross Over, Medication Technician Meetings and others as directed by the Supervisor / Department Coordinator.

Maintains compliance in assigned required training and all training required by state/province or other regulating authorities as applicable to this role to ensure that Sunrise standards are always met.

Experience and Qualifications

High School diploma / GED accepted and may be required per state/provincial regulations

Must be at least 18 years of age

Minimum of one (1) year experience working with seniors in assisted living, home health, independent living, hospital or long term care environment and desire to serve and care for seniors

Medication Management Certified with a successful completion of Sunrise University mediation management training

Demonstrates leadership competencies

Ability to make choices, decisions and act in the resident’s best interest

Ability to react and remain calm in difficult situations

Ability to handle multiple priorities

Possess written and verbal skills for effective communication and level of understanding

In addition to base compensation, Sunrise may offer discretionary and/or non-discretionary bonuses. The eligibility to receive such a bonus will depend on the employee’s position, plan/program offered by Sunrise at the time, and required performance pursuant to the plan/program.
