RN Hospice Nurse Case Manager – Beacon Hospice, Amedisys

As a Hospice RN Case Manager, you will assess patients, collaborate with the care team, and develop individualized plans of care to support patients and families during end‑of‑life care. Benefits
Competitive salary: $90,000.00 – $98,000.00 A full benefits package: choice of affordable PPO or HSA medical plans, paid time off, 401(k) with company match. Family support: infertility treatment coverage, adoption reimbursement, paid parental and family caregiver leave. Fleet vehicle program and mileage reimbursement. Responsibilities
Perform patient assessments and develop and implement individualized plans of care. Collaborate with the care team to coordinate referrals to other hospice disciplines as needed. Maintain accurate, timely patient progress records in accordance with care center policies. Document pain/symptom management changes and outcomes. Educate patients, families, and caregivers on appropriate patient care techniques. Communicate regularly with the clinical manager and care team. Provide on‑call services to patients and families as assigned. Perform other duties as assigned. Qualifications
One year of nursing experience. Current RN license in the state(s) assigned to work. Current CPR certification. Valid driver’s license, reliable transportation, and liability insurance. Our compensation reflects the cost of labor across several U.S. geographic markets and may vary depending on location, job‑related knowledge, skills, and experience.
