Full-time Family Nurse Practitioner Needed at Opti Nurse Consultants in Auburn, NY You will have the opportunity to provide holistic care to individuals and families in the community, addressing their unique healthcare needs and advocating for equitable access to quality services. By collaborating with community organizations and implementing health education initiatives, you will play a pivotal role in empowering individuals and improving community health outcomes. Work Schedule: Schedule from 9:00 a.m. to 6:00 p.m., with occasional evening clinics until 8:00 p.m. Participation in community outreach events or health fairs during weekends may be required. Approximately 40 hours per week. Responsibilities: Conduct health assessments and screenings for individuals and families. Diagnose and manage acute and chronic conditions, including preventive care. Develop and implement health education programs with community organizations. Provide culturally sensitive care and advocate for patient rights and access. Coordinate referrals and follow-up care for continuity. Maintain accurate medical records per standards. Participate in outreach events and health fairs. Stay updated on community health trends and policies. Requirements: Current Nurse Practitioner certification. Active state nursing license. Master's degree in nursing from an accredited program. 3-5 years of clinical experience, preferably in community health. Strong interpersonal and communication skills. Commitment to health equity and social justice. Salary and Benefits: Salary: $180,000-$200,000 annually, based on experience.
