Full-time Family Nurse Practitioner Needed at Opti Nurse Consultants in Rome, NY
You will have the opportunity to provide holistic care to individuals and families in the community, addressing their healthcare needs and advocating for access to quality services. Collaborate with community organizations and lead health education initiatives to empower individuals and improve community health outcomes.
Work Schedule:
Hours from 9:00 a.m. to 6:00 p.m., with occasional evening clinics until 8:00 p.m.
Participation in community outreach or health fairs on weekends may be required.
Approximately 40 hours per week.
Responsibilities:
Conduct health assessments and screenings for community individuals and families.
Manage acute and chronic conditions, including preventive care.
Develop and implement health education programs with community partners.
Provide culturally sensitive care and advocate for patient rights and access.
Coordinate referrals and follow-up for continuity of care.
Maintain accurate medical records per standards.
Participate in outreach and health fairs to raise awareness.
Stay informed about community health trends and policies.
Requirements:
Current Nurse Practitioner certification.
Active state nursing license.
Master's degree in nursing from an accredited program.
3-5 years of clinical experience, preferably in community health.
Strong communication skills and ability to work with diverse populations.
Commitment to health equity and advocating for vulnerable communities.
Salary and Benefits:
Salary between $180,000 and $200,000 annually, based on experience.
