You will have the opportunity to provide holistic care to individuals and families in the community, addressing their healthcare needs and advocating for equitable access to quality services. Collaborate with community organizations and implement health education initiatives to empower individuals and improve community health outcomes. Work Schedule:
Your schedule will range from 9:00 a.m. to 6:00 p.m., with occasional evening clinics until 8:00 p.m. Participation in community outreach events or health fairs during weekends may be required. An average workweek totals approximately 40 hours. Responsibilities:
Conduct comprehensive health assessments and screenings for individuals and families. Diagnose and manage acute and chronic medical conditions, including preventive care and health promotion. Collaborate with community organizations to develop and implement health education programs. Provide culturally sensitive care and advocate for patients' rights and access to healthcare services. Coordinate referrals and follow-up care to ensure continuity. Maintain detailed medical records in compliance with standards. Participate in community outreach events and health fairs. Stay updated on community health trends and evidence-based practices. Requirements:
Current certification as a Nurse Practitioner. Active state nursing license. Master's degree in nursing from an accredited program. Minimum of 3-5 years of clinical experience, preferably in community or public health. Strong interpersonal and communication skills. Commitment to social justice and health equity. Salary and Benefits:
