This full-time community health nurse practitioner position offers the opportunity to provide holistic care to individuals and families, addressing their healthcare needs and advocating for equitable access to quality services. Schedule ranges from 9:00 a.m. to 6:00 p.m., with occasional evening clinics until 8:00 p.m. Participation in community outreach events or health fairs during weekends may be required. The average workweek is approximately 40 hours. Responsibilities
Conduct comprehensive health assessments and screenings for individuals and families. Diagnose and manage acute and chronic medical conditions, including preventive care and health promotion. Develop and implement health education programs in collaboration with community organizations. Provide culturally sensitive care and advocate for patients' rights and access to healthcare services. Coordinate referrals and follow-up care to ensure continuity of care. Maintain accurate medical records in compliance with standards. Participate in community outreach events and health fairs to raise awareness and provide screenings. Stay updated with community health trends, evidence-based practices, and relevant policies. Requirements
Current certification as a Nurse Practitioner. Active state nursing license. Master's degree in nursing from an accredited program. Minimum of 3-5 years of clinical experience, preferably in community or public health settings. Strong interpersonal and communication skills, with ability to work effectively with diverse populations. Commitment to social justice, health equity, and advocating for vulnerable communities.
