Evaluate and analyze assessment information and provide PNMT recommendations for individuals meeting referral criteria or as directed by the interdisciplinary team (IDT). Plan, develop, evaluate, and treat physical and nutritional management issues from the nursing perspective as part of PNMT. Collaborate with the IDT to integrate PNMT recommendations into the Individual Support Plan (ISP) and to optimize care coordination. Complete PNMT Post-Hospitalization Assessments/Reviews and report results to determine continued PNMT needs. Analyze data and develop PNMT recommendations with measurable goals and action plans; assist with IHCP development for individuals on PNMT caseload. Document progress notes and participate in policy development, literature review, and updates to standards of care related to physical and nutritional management. Provide direct nursing care as needed and assist with care for HHS employees injured on the job. Participate in and/or support disaster response, continuity of operations (COOP), and other agency activities as required. Essential Job Functions (EJFs)

Attend regular work shifts and participate in meetings; monitor health status and ensure referrals to PNMT when criteria are met. Participate in PNMT discussions in IDT meetings; lead or support discussions and perform specialized nursing assessments as needed. Develop PNMT recommendations with goals and action steps; help form or update IHCPs and ensure integration into ISP. Review hospital documentation and perform post-hospitalization assessments; coordinate with hospital liaison nurse. Monitor PNMP activities and PNMT interventions; report results in ISPA meetings and other oversight groups. Analyze data, conduct audits, and contribute to policy development; prepare monthly reports on targeted areas. Stay current with literature and training on physical and nutritional management; deliver training as assigned. Provide direct care when required and support safety and health needs for individuals served. Qualifications

Registrations, Licensure Requirements or Certifications:

Licensed to practice as a Registered Nurse in Texas (or state with reciprocity through NLC). Initial Screening Criteria: At least 1 year of experience as a practicing RN required. At least 2 years of RN experience in a health agency, hospital, or health-care facility preferred. Bachelor’s Degree preferred. Experience in assessment and documentation of health status required. Specialized training or experience with individuals with complex physical and nutritional management needs preferred; IDD experience preferred. Additional Information
