Essential Job Functions (EJFs)

Attend work regularly, including rotating on‑call shifts as required.

Participate in state center meetings to monitor individual health status.

Track PNMT referral criteria and intervene with individuals who meet policy thresholds.

Lead or participate in PNMT meetings and interdisciplinary team (IDT) discussions.

Perform specialized nursing assessments and develop PNMT recommendations with measurable goals.

Develop individual health care plans (IHCP) for active PNMT caseload and document progress notes.

Conduct PNMT Post‑Hospitalization Assessment/Review and communicate results to PNMT.

Collaborate with IDT to optimize PNMT recommendations and integrate them into the Individual Support Plan (ISP).

Monitor and evaluate PNMT effectiveness, including medication administration, positioning for enteral feeding, and aspiration prevention.

Provide direct care nursing services for HHS employees injured in scope of employment.

Contribute to policy development, training, and review of PNMT‑related standards.

Assume additional duties, including disaster response, continuity of operations, and on‑call status as needed.

Knowledge, Skills, and Abilities (KSAs)

Extensive knowledge of nursing techniques, standards, and procedures; health care laws and regulations; medical diagnoses and procedures; evidence‑based practices for developmental disability populations.

Proficiency in computer/software use, electronic health record systems, and data graphics.

Excellent verbal and written communication skills with the ability to present ideas clearly.

Ability to interpret and apply state and federal laws, ICF‑IID rules, and regulations.

Strong collaboration skills with interdisciplinary teams and other staff members.

Capability to develop measurable goals, action plans, and recommendations for PNMT interventions.

Physical fitness, CPR certification, and safety procedures for emergency and patient care.

Independent judgment with latitude for initiative and decision making in nursing practice.

Qualifications

License to practice as a registered nurse in Texas (or a state recognized by the Nurse Licensure Compact).

At least one (1) year experience as a practicing registered nurse.

Preferred: Two (2) years experience in a health agency, hospital, or health‑care facility; Bachelor's Degree; experience with individuals with intellectual or developmental disabilities (IDD).

Experience in assessment and documentation of health status required.

Specialized training or experience with individuals with complex physical and nutritional management needs preferred.
