Clinical Case Managers provide comprehensive psychological and behavioral case management services across a continuum of high to low intensity services designed to improve or maintain consumers’ abilities to function in the community. Responsibilities include assessing, monitoring, following up on consumers' ability to meet basic needs, and clinical documentation. Transportation of consumers in your vehicle is required (additional insurance cost reimbursed). Learn more about WellPower: Pay Range & Benefits: CM I : $28.96/hr - $60,237/year (Bachelors Degree) CM II : $30.05/hr - $62,504/year (Master’s Degree) Language Differential: $1/hr - $2.50/hr for bilingual proficiency in Spanish & ASL We are committed to fair salaries based on experience, education, and licensure, within predefined ranges. Details at: https://wellpower.org/workplace-of-choice/ Responsibilities & Duties: Assist consumers in meeting basic needs and applying for benefits; establish protective payee-ship if necessary. Ensure access to services and resources, including transportation (personal vehicle with reimbursed insurance). Provide psychosocial services to improve or maintain functioning, including: Evaluating strengths and symptoms, setting goals, planning services; Teaching daily living skills; Developing social skills and leisure activities; Supporting employment and housing needs; Engaging families, friends, and community resources; Utilize natural support systems and outreach to eligible consumers. Maintain a trauma-informed environment. Other duties as assigned. Requirements & Qualifications: Master’s Degree in psychology, social work, or related field. One year experience in mental health or human services preferred. Valid Colorado driver’s license, clean driving record, ability to transport consumers, and obtain business use auto coverage (reimbursed). Why Work at WellPower?
