Clinical Case Managers Provide comprehensive psychological and behavioral case management services within a continuum of high to low intensity services designed to improve or maintain each mental health consumer’s abilities to function in the community. Responsibilities include assessing, monitoring, and following up with consumers regarding their ability to meet basic needs and move toward recovery. Responsible for all clinical documentation of consumer interactions. As a Clinical Case Manager, you will also provide transportation for consumers in your vehicle (additional insurance costs reimbursed).

- $28.96/hr - $60,237 annually (Bachelor's Degree)

CMII

- $30.05/hr - $62,504 annually (Master’s Degree)

Language Differential

- $1/hr - $2.50/hr for bilingual proficiency in Spanish & ASL

Responsibilities & Duties:

Assist consumers in meeting basic needs and help with applications for income, medical, housing, or other benefits. Identify consumers needing protective payee-ship and arrange this assistance.

Arrange transportation or personally transport consumers to needed services using your vehicle (business insurance reimbursed).

Provide psychosocial services to improve or maintain consumers’ abilities to function socially, including:

Evaluating strengths and symptoms, setting goals, and planning services;

Teaching daily living skills;

Developing social skills and leisure activities;

Assisting with employment or vocational services;

Helping locate supportive housing;

Engaging families, friends, and community resources to maximize benefits.

Identify and collaborate with natural support systems like community organizations.

Encourage treatment engagement by utilizing consumer support systems.

Reach out to eligible consumers, informing and educating them about available services.

Maintain a trauma-informed environment.

Perform other duties as assigned.

Requirements & Qualifications: Education:

Master’s Degree in psychology, social work, or related human services field.

Experience:

One year in mental health or human services preferred.

Must have a valid Colorado Driver’s license, a clean driving record, and the ability to transport clients in your vehicle. Ability to obtain and carry business use auto insurance (reimbursed).
