Texas Children's Hospital . We are searching for a Diabetes Educator -- someone who works well in a fast-paced setting. In this position, you will provide assessment, goal setting, education, behavioral intervention, and ongoing case management to patients and caregivers to promote diabetes self-management in both inpatient and outpatient settings. Qualifications: Bachelor’s degree required; master’s preferred. CDCES from the Certification Board of Diabetes Care and Education OR BC-ADM from the Association of Diabetes Care and Education Specialists required. At least 1 year of experience in specialized diabetes care as a licensed dietitian or nurse. All successful candidates will be required to undergo respiratory fit testing as part of our commitment to a safe workplace. Responsibilities: Teach patient and family skills for diabetes self-management, assessing knowledge and tailoring education based on social, economic, cultural, and age-specific needs. Evaluate patient needs periodically and suggest management strategies, including insulin infusion management (IIM) and diabetes technology. Document all patient care activities according to guidelines. Develop and provide educational materials for staff, patients, caregivers, and the community. Maintain current knowledge through continuing education and evidence-based practices. Implement protocols for diabetes management. Provide case management services, including equipment requests, preauthorization, and care coordination. When appropriate, assess and provide nutritional care, especially if holding an RD/RDN credential. About Us:
