As a Telephonic Nurse Care Manager with Humana Gold Plus-Integrated in Illinois, you will play a pivotal role in transforming the lives of our members by providing personalized assessments, ongoing care planning, and expert guidance over the phone. You will help members overcome barriers to care, manage chronic conditions, and connect with vital resources—empowering them to achieve and maintain their best possible health. Your clinical expertise and compassionate support will have a lasting impact, helping individuals and families navigate complex health challenges and improve overall well-being. Base pay range: $71,100.00/yr - $97,800.00/yr. This range is provided by Humana. Your actual pay will be based on your skills and experience — talk with your recruiter to learn more. Responsibilities

Works with an assigned member population in Illinois over the phone performing health assessments, care planning, and education on chronic conditions and any gaps in their health care plans. Employs a variety of strategies, approaches and techniques to manage a member\'s physical, environmental and psycho-social health issues. Identifies and resolves barriers that hinder effective care. Ensures patient is progressing towards desired outcomes by continuously monitoring patient care through use of assessment, data, conversations with member, and active care planning. Navigates multiple systems and uses critical thinking skills. Required Qualifications

Licensed Registered Nurse (RN) in the state of Illinois with no disciplinary action or ability to obtain IL RN licensure. Minimum of 3 years of clinical acute care experience as a licensed RN. Comprehensive knowledge of Microsoft Office applications including Word, Excel, and Outlook. Preferred Qualifications

Bachelor\'s degree in nursing (BSN). Bilingual in English/Spanish, English/Polish, or English/Creole. Experience with case management, discharge planning and patient education for adult acute care. Managed care experience. Certified Case Manager (CCM). Prior experience with Medicare & Medicaid recipients. Experience with health promotion, coaching and wellness. Knowledge of community health and social service agencies and additional community resources. Additional Information

Workstyle: Remote, work from home. Work Location: Illinois preferred. Typical work days/hours: Monday – Friday, 8:00am – 5:00pm CST, occasional flexibility in scheduling may be required to accommodate member needs. Travel: Occasional travel to Humana\\'s offices for training or meetings may be required. Internet & Equipment

To ensure home or hybrid home/office employees can work effectively, internet service must meet minimum criteria: download speed 25 Mbps and upload speed 10 Mbps; wireless, wired cable or DSL connection recommended. Humana may provide equipment as needed to meet business requirements. Work from a dedicated space free from ongoing interruptions to protect member PHI / HIPAA information.
