Nurse Case Manager – Arbor Hills

Responsibilities
Develop, evaluate, and implement individualized health care plans for patients.
Assess new patients and review test results from medical examinations.
Implement physician orders and follow up with treatment plans.
Communicate treatment plans between families, doctors, and patients.
Document medical records and provide emotional support to families and patients.
Provide education about health care plans to patients and families.
Qualifications
Minimum of a Nursing Degree from an accredited college or university.
At least 1 year of supervisory experience in a hospital, nursing care facility, or related health care facility.
Training in rehabilitative and restorative nursing practices.
Strong communication, excellent customer service, teamwork, and organizational skills.
Current, unencumbered, active RN license in California.
Knowledge of nursing and medical practices, laws, regulations, and guidelines applicable to nursing care facilities.
Ability to deal tactfully with personnel, residents, family members, visitors, government agencies, and the general public.
