Responsibilities
Evaluate and update patient health care plans
Participate with the facility IDT to ensure the resident plan of care is maintained and followed
Proficient in Medicare Rules, Regulations and RAI assessment skills
Complete required skilled charting for residents from admission until discharge from skilled services
Review medical forms, reports, evaluations, etc., for utilization, completeness and accuracy
Act as a liaison with Case Managers
Create plan of care for discharges
Round with physicians when necessary
Qualifications
Must be an LVN with an active, unencumbered license to practice in CA
Must have, as a minimum, 2 years of experience in a skilled nursing facility
Knowledge of nursing and medical practices and procedures, and of laws, regulations, and guidelines pertaining to nursing care facilities
