Overview
Community Health Worker - Birthing Families Navigation role at Erie Family Health Centers. The CHW supports Birthing Family Navigators in maintaining client service data in electronic health records and funder data management systems. This role requires flexibility with schedule and site, including working every other Saturday, to support Birth Families services across Erie. Responsibilities
Work with team leadership in monitoring Erie procedures in programs as assigned. Enroll referred clients in appropriate program and manage enrollments in electronic medical record and funder data management systems in accordance with agency policies and federal, state, and city requirements. Under direction from team leadership, prepare and provide documentation (e.g., statistics) for program reports, including the 745 reports. Create client hardcopy charts and complete filing. As needed, support Birthing Families Navigators with meeting patient needs (e.g., assisting patients who speak languages other than English or Spanish, connecting patients to additional resources). Qualifications
Education
Required:
High School diploma or equivalent. Preferred:
Associate degree or HS Diploma or equivalent with 1-2 years of work experience in customer service or data entry. Community Health Worker Certification a plus. Experience
Preferred:
Experience with data entry; experience in healthcare or social services. Skills And Knowledge
Required: Cultural competency to work in a low income, minority environment. Bilingual fluency (English/Spanish) as required by site/population; may assist or work with non-English/Spanish-speaking patients. Attention to detail with data and reports.
