Responsible for the direct and indirect nursing care of the patients in the home. In collaboration with the patient, family, and health care team (IDT), the registered nurse as case manager will develop, implement, and evaluate nursing care according to agency and system policies. Performs the functions of the home health aide and monitors basic patient care according to policies and procedures. Provides a safe and comfortable environment in the patient’s home; ensures privacy and demonstrates concern for the dignity of the patient in performing all patient care. Completes assignments in a timely manner and assumes on-call responsibility per policy and procedures. Communicates patient care problems or potential problems to the Administrative Staff and/or other interdisciplinary team members to assure continuity and coordination of care. Provides care based on patient age-specific criteria and education/standards; completes OASIS SOC/ROC, OASIS Recertification, and Discharge Oasis per guidelines. Manages admission criteria per agency guidelines and conducts full system assessments at admission and during each visit. Reports significant observed changes in patient condition and documents patient status and care in appropriate areas per policy and procedure. Coordinates care to ensure medications are administered as ordered and aligns with medication administration policies. Maintains IV site integrity and demonstrates proper bag technique; uses equipment per policy and procedure; supports cost containment through efficient use of supplies. Performs treatments and procedures per standards (e.g., vital signs, wound care, ostomy care, catheterization, suctioning, tube feedings, etc.) and provides patient teaching as required. Identifies and facilitates changes in level of care with the care team as appropriate. Recognizes stages of death and dying and documents care and plans accordingly. Qualifications
Education, Certification, and/or Licensure
Current Registered Nurse license issued by the state in which services will be provided or current multi-state Registered Nurse license through the enhanced Nurse Licensure Compact (eNLC). Valid driver’s license. CPR certification within 30 days of hire date. Experience – Preferred Qualifications
One year of Home Care Nursing and two years of prior nursing experience. Physical Requirements
Frequent walking, standing, stooping, kneeling, reaching, pushing, pulling, lifting, grasping, and feeling are necessary. Must be able to lift, move, and position patients of all weights; read and write legibly in English; hearing and sight within normal ranges. Exerting 20 to 50 pounds of force occasionally, 10 to 25 pounds frequently, and up to 10 pounds constantly; physical demands exceed those for light work. Must be able to operate a motor vehicle safely. Working Environment
Exposure to bodily fluids, tissues, contagious diseases, sharps, and potentially hazardous environments. Exposure to toxic gases, fumes, odors, high stress, electrical hazards, and radiation from x-rays. Majority of work performed in patients’ homes with extensive travel; may be exposed to unpredictable or adverse home environments. Skills and Abilities
Ability to perform concentrated and complex mental activity and work well under stress. Strong independent judgment and collaboration with a multidisciplinary team. Ability to follow oral and written instructions; strong written and verbal communication; math skills for dosages and infusion stats. Willingness to work weekends, holidays, and all shifts; adherence to safety policies and IV skills. Additional Job Details
