Overview
Optum is seeking a NP to join our HouseCalls team in Adair, Macon Counties, MO. Optum is a clinician-led care organization, that is creating a seamless health journey for patients across the care continuum. In this role you will provide annual clinical assessments to patients in the comfort of their homes. This is a Part Time position (20 hours per week). HouseCalls is a Nurse Practitioner led model of care designed around the person, offering a fulfilling career where you can focus on keeping patients healthy. The HouseCalls team works to reduce health risks, coordinate follow-up care, and connect members to key support services. The role supports flexible scheduling across a 7-day work week with no on-call, weekends, or holidays required. Primary responsibilities include clinical assessments in-home, identification of care gaps, and coordination with primary care providers to improve health outcomes. Base pay
$89,900.00/yr - $160,600.00/yr $20,000 Student Loan Repayment or $10,000 Sign-on Bonus
for individuals who have not previously participated in this program. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc.
In addition to salary, a comprehensive benefits package is offered; includes incentive and recognition programs, equity stock purchase and 401k contributions (subject to eligibility requirements). Responsibilities
Conduct in-home assessments including evaluation of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical examination Perform evidence-based practice screenings including point of care testing (as appropriate) Identify diagnoses to be used in care management and active medical management Formulate a list of current and past medical conditions using clinical knowledge, judgment, and assessment findings Communicate assessment findings to inform the member's PCP of potential gaps in care Identify urgent and emergent situations and intervene appropriately Educate members on disease processes, medications, and adherence Required Qualifications
Active NP license or ability to obtain by start date; unencumbered, with no open/unresolved disciplinary actions Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care, or ability to obtain national certification and/or NP license in state of assignment by start date Ability to perform physical requirements of the job, including moving a 30-pound bag, navigating stairs and diverse dwelling conditions, and performing physical assessments Reliable transportation to complete home visit assessments Preferred Qualifications
