Job Summary

The Hospice RN Weekend Case Manager provides skilled care and case management to clients/families in accordance with the State Nurse Practice Act and accepted professional hospice standards of practice. The Hospice RN Case Manager provides services utilizing nursing theory and processes, communicates/documents observations and assessments, and promotes and maintains quality of life for clients/caregivers through direct care, teaching, and counseling in accordance with the interdisciplinary team care plan.

Education

Other Certificate/Diploma Nursing Required Or Bachelor's Degree Nursing Preferred

Licenses and Credentials

Class D Passenger Vehicle Driver's License [State License] - Generic - HR Only required Registered Nurse [RN - State License] - Generic - HR Only required

Experience

Minimum of one (1) year medical/surgical nursing experience required Hospice/home care experience preferred

Knowledge, Skills And Abilities

Graduate of an approved, accredited school of professional nursing required. Demonstrated excellent verbal and written communication skills required. Demonstrated strong interpersonal, organizational, and time management skills required. Demonstrated knowledge and understanding of the dying process with an ability to identify and treat signs and symptoms of the same including competency in pain and symptom management required. Good driving record, and reliable transportation required.
