We currently have a position for an experienced Home Health Care Registered Nurse. Candidates with ICU, PICC Line, Wound Care, and Infusion experience are highly preferred.
The corporate office is located in Southfield, MI, and the individual will provide RN support throughout the Tri-County area (Oakland, Macomb & Wayne).
Qualifications:
Graduate of an approved nursing program and licensed in Michigan.
At least one year of nursing experience.
Kinnser - EMR experience preferred.
Home Healthcare experience preferred.
Experience with wound vac, PICC line, and mediport care is a plus.
Pediatric experience is desirable.
Responsibilities:
Provide services according to the plan of care.
Conduct initial and regular reevaluations of patients' nursing needs.
Initiate and revise the plan of care as needed.
Deliver specialized nursing services and preventive/rehabilitative procedures.
Document patient visits and care conferences promptly.
Coordinate services and communicate patient condition changes.
Counsel patients and families on nursing and related needs.
Participate in and lead in-service programs.
Follow agency policies and procedures.
Process orders, notify physicians, and complete certifications and discharge summaries.
Assess nursing needs and refer to specialists when necessary.
Supervise and educate other nursing staff.
Conduct patient care conferences and participate in quality improvement activities.
Handle on-call duties during nights, weekends, and holidays.
Complete required assessments and documentation within specified timeframes.
Utilize ICD-10 codes appropriately.
Working Environment: Indoor work at agency office and patient homes, with travel within the service area.
