Essential Duties And Responsibilities
Perform triage, patient education, resulting of abnormal labs, transitional care management, administering medications, and addressing patient messages beyond CMA/MA scope.
Deliver patient care based on practice guidelines, standards of care, and applicable federal/state laws and regulations.
Accurately document all interactions in the patient’s medical record.
Collaborate with internal healthcare team members to assess, plan, implement, and evaluate patient care.
Coordinate with external providers, hospitals, and case managers to ensure continuity of care.
Prepare patients for provider encounters, including chart audits.
Assist providers with procedures and minor surgeries.
Maintain tracking systems to ensure compliance with FHLC policies, medical guidelines, and health maintenance standards.
Provide patient triage and rooming as required.
Participate in multidisciplinary teams to improve care processes and outcomes.
Perform diagnostic and preventive procedures with technical competency under FHLC protocols.
Mentor and provide clinical support to nursing and support staff (LPNs, CMAs, MAs).
Demonstrate strong customer service skills to support patients and de-escalate difficult situations.
Administer medications under the direction of FHLC practitioners.
Manage and complete ER follow-ups.
Implement infection control protocols and participate in quality improvement initiatives.
Uphold patient confidentiality and compliance with FHLC policies.
Regular and reliable attendance.
Perform other duties as assigned.
Minimum Qualifications
Work independently, adapt under pressure, and demonstrate critical thinking and problem-solving skills.
Respect patient confidentiality while performing job duties.
Build and maintain effective working relationships with patients, staff, and the community.
Education & Experience Requirements
Current State of Wisconsin Registered Nurse License or Nurse Compact License (required).
Outpatient clinic experience, preferably in Family Medicine.
3–5 years of nursing experience (preferred).
Supervisory experience with interdisciplinary teams (preferred).
Knowledge of QI program implementation and maintenance (preferred).
Proficiency in MS Office and EPIC.
