Care Initiatives
Care Initiatives Hospice invites a compassionate Registered Nurse (RN) to join our team as a Hospice RN – Case Manager. In this role you will provide comprehensive, patient‑centered care across home, assisted living, or hospital settings, ensuring dignity and support for patients during their final stages of life.
Responsibilities
Use clinical nursing knowledge, physical assessment, and procedural skills to deliver high‑quality patient care at the patient’s place of residence.
Develop and oversee the plan of care under the direction of the physician and ensure appropriate interdisciplinary involvement to achieve the best outcomes.
Perform initial and comprehensive assessments on all patients in accordance with policies and procedures.
Conduct continuous visits and interdisciplinary assessments of patients.
Deliver quality care and complete clear, concise, and accurate clinical documentation.
Provide patient, caregiver, and family counseling and education as indicated in the plan of care.
Develop and continually update the plan of care to identify the services required to meet patient‑specific needs.
Qualifications
Active Iowa RN license.
Active CPR certification.
Hospice or palliative care experience.
Two (2) years of verifiable nursing experience (RN/LPN), preferred.
Current driver’s license, automobile liability insurance, and ability to travel between multiple locations within your region.
Home access to the internet.
Ability to obtain Hospice and Palliative Registered Nurse Certification (CHPN) within 24 months of employment.
