Overview
Acute Nurse Practitioner, Family Nurse Practitioner or Physician's Assistant – Kidney Care Center of Georgia, LLC is expanding its nephrology team in Northeast Georgia. We are seeking a motivated Advanced Practice Provider (APP) to join our clinical practice to provide patient-centered, high-quality care to patients with kidney disease and related conditions. Responsibilities
Provide inpatient care, including patients in the intensive care units, with initial evaluation and management, follow-up, and dialysis services (hemodialysis). Provide office care for ongoing treatment of CKD and post-hospital follow-up, and participate in in-center dialysis visits for End Stage Kidney Disease patients. Document and bill per requirements at all locations. Coordinate plan of care, discharge planning and specialty referrals as appropriate to meet age-specific needs. Perform comprehensive physical examinations and assessments; interpret and evaluate medical records and social data. Perform and/or assist with procedures per collaborative agreement; participate in performance improvement activities. Maintain ongoing communication with collaborating physician for consultations or arrange for pre-approved medical coverage as required by state regulations. Adhere to the established scope of practice and provide education to patients, families and other healthcare providers as needed. Work Schedule
Monday–Friday, 8:00 am–5:00 pm with weekend coverage (maximum once per month). Position Requirements
Master's degree in Nursing with Acute Care NP certification or graduate from an accredited Physician Assistant program. Currently certified as an Advanced Practice Nurse (APN) with CRNP specialty by the Georgia Board of Nursing or licensed as a Physician Assistant by the Georgia Board of Medical Examiners with PA-C certification by the NCCPA.
