Description: PHYSICAL THERAPIST - Home Health Services - Seaside, Oregon. Schedule: Full time, Day Shift. A $7,500 hiring bonus is available for eligible external hires meeting required qualifications and conditions for payment. Responsibilities

Manages and provides skilled physical therapy to patients/clients with a wide variety of diagnoses and disabilities in a home setting in accordance with a physician plan of treatment, taking into account individual patient special or age-related needs. Requires skills in patient/client evaluation and treatment, as well as professional consultations to achieve maximum patient/client independence in accordance with the individual\u2019s functional level and potential. Bachelor\u2019s Degree From an accredited Physical Therapy Program. Or Master\u2019s Degree From an accredited Physical Therapy Program. Or D.P.T. From an accredited Physical Therapy Program. Within 30 days of hire: National Provider BLS - American Heart Association. Upon hire: Oregon Physical Therapist License. 1 year Clinical experience in Home Health or healthcare setting, or active enrollment in the Providence postgraduate preceptor program.
