PHYSICAL THERAPIST - Home Health Services - Seaside, Oregon Schedule:
Full time, Day Shift
Manages and provides skilled physical therapy to patients/clients with a wide variety of diagnoses and disabilities in a home setting in accordance with a physician plan of treatment, taking into account individual patient special or age related needs. Requires skills in patient/client evaluation and treatment, as well as professional consultations to achieve maximum patient/client independence in accordance with the individuals functional level and potential.

Required Qualifications
Bachelor's Degree From an accredited Physical Therapy Program. Or Master's Degree From an accredited Physical Therapy Program. Or D.P.T. From an accredited Physical Therapy Program.
Within 30 days of hire: National Provider BLS - American Heart Association.
Upon hire: Oregon Physical Therapist License.
1 year Clinical experience in Home Health or healthcare setting, or active enrollment in the Providence postgraduate preceptor program.

About The Team Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint. HCC provides a full range of services and support for people of all ages, especially those facing chronic or life-limiting illnesses. Our core competencies include restorative care, longitudinal care and care through the end of life.
