Overview
Registered Nurse opportunity focused on wound, ostomy, and continence care in a Long Term Acute Care setting. Provides comprehensive assessments, develops care plans, collaborates with the health care team, and supports staff education and quality improvement efforts. Responsibilities
Conducts health assessment interview focused on wound, ostomy, and continence related care. Performs focused physical assessment and develops plan of care based on diagnosis and individual client needs. Recommends medications and treatments as appropriate. Develops, revises, and evaluates standards of care; assists in selection of supplies, technology, and equipment. Assesses and documents patient response to wound care, ostomy, continence, fistula, and complex wound treatments. Evaluates quality and delivery of care provided by clinical staff on the unit. Collaborates with the health care team and makes referrals to other disciplines as indicated. Teaches, counsels, and assists individuals and families to prevent complications related to ostomies, wounds, or continence problems. Develops, provides, and evaluates staff education programs (e.g., new employee orientation, in-services, continuing education). Routinely provides comprehensive assessments of patients at risk for developing pressure injuries and initiates preventative measures. Escalates and communicates challenges, including ethical issues and conflict resolution concerns. Collaborates with the Assistant Director of Nursing for Wound Care to initiate and recommend best practices based on prudent assessment. Participates in Unit/Department Quality Improvement and Hospital-wide QA activities (e.g., incidence, prevalence studies, nursing quality). Provides culturally competent education to patients, families, and staff in alignment with diverse beliefs and practices. Maintains security and confidentiality of data in accordance with patient rights policies. Maintains professional competency by engaging in activities consistent with clinical practice and population served. Identifies community resources helpful to clients and assists with utilization. Asset management related to rented equipment (e.g., specialty beds, negative pressure devices). Qualifications
2 years of medical/surgical experience with wound and colostomy experience preferred. Education
New York State RN License Primary Source Verification AHA BLS Current Certification
CWOCN, CWON, CWCN, CWS, or WCC via respective boards as listed in the original description. Schedule
