Overview
Case Manager III - Shared (On-Call) - overnights at Heartland Family Service. This role serves as a direct care staff/case manager in several residential programs that serve clients who are trauma survivors, those who struggle with substance abuse or mental illness, and victims of intimate partner violence. Programs include substance abuse treatment centers for women with children, emergency shelters for survivors of domestic violence and human trafficking, and a short-term housing program for individuals with mental health and substance abuse disorders. This position provides supervision and support to residents as part of the 24-hour direct care staff team in six residential programs. Summary Description Of Work
This case manager will serve as a direct care staff/case manager in several residential programs at Heartland Family Service that serve clients who are trauma survivors, those that struggle with substance abuse or mental illness, and victims of intimate partner violence. These programs include substance abuse treatment centers for women with children, emergency shelters for survivors of domestic violence and human trafficking, and a short-term housing program for individuals with mental health and substance abuse disorders. This position provides supervision and support to residents at their assigned location as part of the 24-hour direct care staff team in six residential programs. Compensation
$25.00 per hour Work Schedule
Monday through Sunday, overnights Minimum Qualifications
Bachelor’s Degree in Social Work or Human Service field, and two years direct care experience in Human Services preferred; or equivalent employment experience may be considered Valid Driver’s License and acceptable driving record Essential Duties And Responsibilities
Provides supervision and support for program participants as a part of residential program staffing for 24 hour per day, 7 day per week schedule. Responds to all crisis calls, assesses situation, gathers information and provides appropriate and accurate referrals and information, maintaining strict confidentiality. Assist with meeting basic needs in several residential programs; including assistance with developing and revising an ongoing safety plan, and connecting to medical, therapeutic, or other providers as identified through the intake and assessment process. Provide client transportation in personal and agency vehicles. Follow program policies and procedures; including agency protocols related to medication administration. Documents client activity, progress and needs per established record keeping procedures. Responsible for maintenance and upkeep of facility, including monitoring residents’ activities. Works as a team member with other program staff to provide trauma-informed care to clients. Always displays a courteous and caring attitude to the clientele, volunteers, and visitors of the Agency. Is dependable and punctual regarding scheduling and attendance. Able to work independently with little supervision. Agree to work in at least two different programs for a minimum of three shifts per month. Maintains required records and statistical information. Strives to make connections between the agency and the larger community whenever possible to contribute to the agency’s ongoing fundraising and friend-raising efforts. Creates, maintains, and shares an appropriate and dynamic self-care plan. Essential functions of this job are to be performed on company physical work site.
