Overview
Eventus WholeHealth is hiring for a Nurse Practitioner or Physician Assistant in Primary Care. Eventus WholeHealth delivers an integrated model of care to adults residing in skilled nursing and assisted living facilities through a network of healthcare providers including Physicians, Nurse Practitioners, Physician Assistants, Medical Assistants and in-house Support Staff. Responsibilities
Provide primary-care services in accordance with Eventus WholeHealth site-specific protocols and in line with the standard of care for family medicine, geriatric medicine, and/or internal medicine as it pertains to long term care facility work. Review and sign MARs, sign orders specific for Primary Care while at the facility and/or within 24 hours if faxed or emailed, including phone orders, DME orders, Death Certificates, Plans of Care, PT/OT orders, hospice orders as appropriate, and other orders relevant to patient care. Review and address labs within 24 hours of receipt. Manage narcotic prescriptions as per Eventus WholeHealth Narcotic Rx Policy. Allocate adequate time during each visit for administrative duties. Complete documentation and submit billing records according to published policies. Provide the facility with a hard copy of electronic documentation within 48 hours of service for inclusion in the patient’s facility medical record. Maintain open and positive communications with facility staff and administration. Provide instructions or feedback regarding medications and other pertinent caregiver information. Meet with the Administrator or designated facility care coordinator prior to the start of patient visits each day and prior to leaving the facility each day. Be knowledgeable of and adhere to Eventus WholeHealth standards, policies and procedures. Comply with all applicable legal, regulatory, and ethical standards governing health care delivery. Maintain patient confidentiality at all times, including appropriate use of cell phones, emails, text messaging, patient charts and EMR. Abide by HIPAA rules and regulations. Consult with the clinical supervisor before refusing a resident, family, or facility request or recommendation regarding care. Qualifications/Education
Masters degree in Nursing from an accredited institution and a current and valid license to practice medicine in the state of residence. All other certifications necessary to perform duties must be current and valid. Or, graduation from a physician assistant program accredited by the National Commission on Accreditation of Educational Programs, with passage of the national certification examination and licensure as a Physician Assistant in the state of residence. Specialization in geriatric, family, or psychiatry is desirable. About Eventus WholeHealth
