Job Overview
The LPN Medication Care Manager is responsible for providing the highest degree of quality care and services by administering medication and treatments in a safe organized manner. Responsibilities include administration of medications, documentation of medication administration, conducting SHUs, and providing resident care while demonstrating the Mission for Sunrise Senior Living, to champion the quality of life for all seniors in accordance with federal, state/provincial, and local laws, standards and regulations and Sunrise Senior Living policies to promote the highest degree of quality care and services to our residents. Responsibilities
Medication Administration: Provide the highest degree of quality care and services by administering medication and treatments in a safe, organized manner. Documentation: Review, read, notate, and initialize Daily Log to document pertinent information about residents; receive medication updates from the Resident Care Director (RCD) or Wellness Nurse; administer, assist with, and observe medications and treatments; ensure documentation is completed for all doses, refusals, or missed doses. Medication Safety: Pass medications according to the prescribed times; maintain confidentiality of resident information including medication data; restock medication carts; assist in checking medication packaging; count narcotics with another MCM or Lead Care Manager each shift. Clinical Assessment: Assess residents to determine need for as-needed medications and appropriately document and report to supervisor; ensure clean and organized Medication Room, carts, and treatment carts; follow the refill process for medications. Compliance and Safety: Follow Sunrise principles of service; maintain hygiene and safety standards; adhere to infection control techniques and safety procedures; practice safe handling of blood-borne pathogens, hazardous materials, and PPE; ensure oxygen tanks are stored safely and used per prescribed levels. Communication and Coordination: Report resident concerns to the RCD or Wellness Nurse; notify RCD of concerns through LEAD process; participate in daily Cross Over meetings; transcribe orders as directed; participate in ISP/ICP development and monthly updates. Attend and actively participate in daily Cross Over meetings; contribute to Service and Health Updates as directed by RCD; participate in the development of ISP/ICP and monthly updates; transcribe orders. Risk Management And General Safety
Ensure community compliance with OSHA and applicable safety regulations; promote risk management programs; use PPE, SDS, and lockout/tagout procedures; report accidents and unsafe conditions immediately; keep exits clear and in compliance with fire codes; follow infection control techniques and disposal procedures; report occupational exposures immediately. Ensure safe storage and handling of oxygen tanks and other hazardous materials; monitor prescribed oxygen levels. Training And Team Contribution
Actively participate as a team member; uphold Team Member Credo and Principles of Service; contribute to engagement programs and attend required meetings and trainings; may be designated as shift supervisor or supervise other MCMs; maintain required training compliance. Qualifications
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed are representative of the knowledge, skills, and abilities required. LPN, LVN, or state/provincial licensed nurse credential; appropriate certifications must be maintained where applicable (CPR and First Aid). Minimum age 18; prior experience with seniors preferred; strong desire to serve and care for seniors; ability to make independent, resident-centered decisions; ability to remain calm under pressure; ability to manage multiple priorities; strong written and verbal communication; good judgment, problem solving, and decision-making skills.
