Sunrise Senior Living is seeking an LPN Medication Care Manager to provide high-quality care and services by administering medications and treatments in a safe, organized manner. This role requires adherence to federal, state/provincial, and local laws, Sunrise policies, and the Sunrise Mission to champion the quality of life for all seniors.
Overview The LPN Medication Care Manager is responsible for administering medications, documenting medication administration, conducting SHUs, and providing resident care while upholding Sunrise's mission and standards.
Responsibilities
Provide the highest quality care by administering medications and treatments safely and in an organized manner.
Review, read, note, and document information about residents in the Daily Log.
Receive medication updates from the Resident Care Director (RCD) or Wellness Nurse.
Administer, assist with, and observe medications using the medication administration record and the Six Rights of Medication Pass (Right resident, medication, dosage, time, route, right to refuse).
Ensure medications are passed as scheduled and document refusals or missed doses.
Maintain confidentiality of resident information concerning medications.
Report resident concerns to the RCD or Wellness Nurse.
Restock medication carts after passes and assist in checking medications, regardless of packaging system.
Assess residents for as-needed medications and document/refer to supervisor as appropriate.
Count narcotics with another Medication Care Manager or Lead Care Manager each shift.
Maintain and clean the Medication Room, carts, and treatment areas; manage inventory and dispose of expired medications.
Follow the refilling process for medications.
Support resident independence and dignity while adhering to Sunrise Principles of Service.
Demonstrate empathy and adapt to each resident's communication patterns, history, and needs.
Maintain standard infection control practices and safety precautions.
Audit medication carts as part of ongoing quality practices.
Resident Care
Notify the RCD of any resident or family concerns through the LEAD process.
Attend and participate in daily Cross-Over meetings facilitated by the LCM.
Conduct Service and Health Updates as directed by the RCD.
Participate in developing ISP/ICP and monthly updates; transcribe orders as needed.
Risk Management and General Safety
Collaborate with the community team to ensure compliance with OSHA, workers' compensation, and safety regulations.
Follow safety procedures including PPE, fire safety equipment, SDS, and Lockout/Tagout where applicable.
Report all accidents/incidents and unsafe conditions immediately.
Keep cords, carts, equipment, and hazards out of walkways and in compliance with fire codes.
Follow infection control techniques and proper disposal of hazardous materials.
Understand and practice safe handling of bloodborne pathogens and other hazards; report exposures immediately.
Ensure oxygen tanks are stored safely and maintained at prescribed levels.
Training and Team Contribution
Act as a collaborative team member and work toward shared goals.
Demonstrate Sunrise Team Member Credo in daily interactions.
Support residents and guests through Sunrise Principles of Service.
Engage in team engagement initiatives, surveys, and improvement planning.
Attend regular meetings and training as directed; may serve as shift supervisor or supervise other MCMs.
Maintain compliance with required trainings and regulatory standards.
Qualifications
LPN, LVN, or state/province-specific licensed nurse credential.
Maintain appropriate certifications as required by state/provincial regulations; CPR and First Aid.
Must be at least 18 years of age. Prior experience with seniors preferred.
Ability to make resident-centered decisions, stay calm in difficult situations, and handle multiple priorities.
Strong written and verbal communication and good judgment/problem-solving skills.
If applicable, drivers who operate a Sunrise vehicle must sign the Driver job description and follow safety/regulatory duties.
