Utilize MSW education and experience to perform telephonic and onsite assessments of medically complex pediatric recipients in the PDN care program.

Work as part of a multidisciplinary team, meeting regularly to develop comprehensive care plans based on patient and family needs.

Use independent judgment to evaluate and modify care plans as needed, communicating changes to families, healthcare teams, and other involved agencies.

Coordinate with families and healthcare teams to arrange for identified home care services.

Consult on patient care issues within areas of expertise and serve as a resource to the community, documenting all activities appropriately.

Manage daily workload related to the quality review process, including case assignments and follow-up to meet contractual timelines.

Prioritize requests professionally to foster cooperative relationships and ensure confidentiality.

Maintain tracking systems for correspondence in accordance with contractual requirements.

Review documentation and prepare outgoing communications to physicians and providers as per policies.

Assist in coordinating Physician Peer to Peer conferences as needed.

Review incoming referrals and peer determinations, escalating questionable cases to management.

Act as an expert resource for external customers.

Participate in ongoing program development, implementation, and evaluation, providing recommendations for improvements.

Contribute to internal initiatives and assist in data gathering for CMS reporting.

Support training of nurses and coordinators on processes and relevant issues.

Provide courteous, prompt service to all customers and identify opportunities for process and service improvements.
