Bluebonnet Trails Community Services . The Collaborative Care Case Manager (CCCM) will provide services for up to 15 individuals in Bluebonnet Trails Community Services Outpatient Biopsychosocial Services for Persons with IDD Program (OBI). The program is designed to assess, offer treatment services, and make referrals to other needed services and support in a biopsychosocial framework. Duties include case management, collaboration with mental health providers, supporting individuals with IDD and their families, addressing barriers to access, and developing person-centered, strength-based treatment plans. The CCCM will facilitate timely follow-up, discharge planning, and diversion when individuals are hospitalized or incarcerated. Services can be provided via tele-video, face-to-face, or telephone, depending on individual needs. The role requires sensitivity to cultural differences and ability to build relationships within and outside the agency. The CCCM will also participate in data collection and reporting as directed. $21.46 - $30.21 hourly Differential Details:

Minimum Qualifications:
Bachelor’s degree in social, behavioral, or human services field and at least one year of experience working with individuals with intellectual disabilities. Valid Texas driver’s license required. Preferred Qualifications:
Two-three years of experience with individuals with intellectual disabilities, mental health, crisis services, bilingual abilities, substance use disorder services, case management experience. Additional Information:
