in the

area, serving adult and geriatric clients. As a home care Physical Therapist, you will be a vital part of a multidisciplinary team delivering rehabilitative care and skilled nursing, enabling clients to remain at home. Performing diagnostic tests such as mobility, joint range, stability, ambulation patterns, strength, endurance, balance, and safety assessments. Implementing wound prevention and repair interventions using evidence-based dressings and topical agents. Developing and revising individualized care plans, including manual exercises, gait training, and balance interventions. Assessing and updating care plans, collaborating with nursing staff as needed. Educating clients and families on rehabilitative activities to promote health, safety, and independence. Documenting observations and interventions accurately via electronic tablets. Qualifications: Current

VA

Physical Therapist license. At least one year of recent PT experience. Graduation from an accredited program by the American Physical Therapy Association or equivalent. Ability to work independently and manage time effectively. Strong interpersonal, organizational, and problem-solving skills. Proficiency with computers; EMR experience preferred.
