is seeking an experienced
Physical Therapist (PT)
for a
per diem weekend
home health position in our Wilmington, DE office. The office serves adult and geriatric clients across
New Castle County . Availability for two weekends per month is required. Home health experience and a minimum of one year of clinical experience as a licensed Physical Therapist are necessary. The PT will be part of a multidisciplinary team providing rehabilitative care and skilled nursing, enabling clients to remain at home. We value our employees highly and invest in their growth. Performing diagnostic tests such as mobility, strength, balance, and safety assessments. Implementing wound care interventions using evidence-based dressings and topical agents. Developing and revising individualized care plans, including therapeutic exercises and gait training. Assessing and updating care plans, collaborating with nursing staff. Educating clients and families on rehabilitative care to promote health and independence. Documenting care activities accurately using electronic tablets. Qualifications: Current
DE
Physical Therapist license. At least one year of recent clinical experience as a PT. Graduation from an APTA-approved program or equivalent. Ability to work independently and manage time effectively. Strong interpersonal, organizational, and problem-solving skills. Proficiency with computers; EMR experience preferred.
