The practitioner will work Monday through Friday 9am – 5pm. No nights, No weekends and no call. Practitioner to work 2 days per week with an ACT team (most of this work in field based) and 3 days per week in an office based setting providing clinical services to seriously mentally ill clients who have history of multiple hospitalizations and non-compliance with traditional treatment and homelessness as part of the interdisciplinary team. Scope of services provided include: Conducting Psychiatric Assessments and Evaluations Prescribing, Monitoring and Administering Psychotropic Medications Provide crisis intervention services to clients Participating in clinical case conferences, team meetings, High Risk conferences, quality improvement initiatives, evidence-based practice implementation and policy/procedure development. The practitioner will also complete all clinical documentation as required by payers, funders, and regulatory authorities via an Electronic Health Record and Complete all administrative paperwork required.
