9:00 a.m. to 6:00 p.m., occasional evening clinics until 8:00 p.m., weekends for outreach events (approx. 40 hours/week) Responsibilities
Conduct health assessments and screenings for individuals and families Manage acute and chronic conditions, promote preventive care Develop and implement health education programs with community partners Provide culturally sensitive care and advocate for patient rights Coordinate referrals and follow-up care Maintain accurate medical records in compliance with standards Participate in community outreach and health fairs Stay updated on community health trends and policies Requirements
Current Nurse Practitioner certification Active state nursing license Master's degree in nursing from an accredited program 3-5 years of clinical experience, preferably in community health Strong interpersonal and communication skills Commitment to health equity and social justice Salary & Benefits
